On 25 January 2010 an Ethiopian Airlines [EAL] flight 409 took off from Lebanon and few minutes later crashed into the sea killing all 90 passengers most of whom were Lebanese. It did not take the Lebanese authorities long to conclude "pilot error" as cause of crash. The evidence they had at the time was words of a kiosk attendant, a gas attendant, a cab-driver who drove the pilots to the airport and who did not speak a word of English or Amharic but still overheard the pilots' premonitions, educated guesses by Lebanese journalists, interpretation of the premonitions and a piece of the debris reclaimed from the sea. Another interesting fact is the number of interviews and communiques Lebanese authorities put out in total disregard for investigative ethics and procedures. Few months later this unrelated but 'normal' incident reported with intent to strengthening the "pilot error" thesis:

"The plane asked to return to Beirut Airport shortly after take- off due to a malfunction," a Lebanese Airport official said. On January 25, 2010, an Ethiopian airliner crashed into the sea near Beirut airport shortly after take off, killing 90 passengers on board, most of whom were Lebanese.
The only conclusion we can come up with is that the Lebanese report has no credibility. One does not begin with a predetermined conclusion and work toward making pieces of evidence fit that conclusion.
